Overview
When your child’s school or club sends you a form — a health action plan, a medical consent form, or similar — you can find it, fill it out, and track its status from one place: the Forms tile on your mymeddiCAL home screen.Finding your forms

Open the Forms tile
On the home screen, tap the Forms tile. It shows a count of how many forms still need your attention.

Or tap the notification
A bell notification is sent whenever a new form is assigned to you — tapping it takes you straight to the Forms page.
To complete
Forms you haven’t started yet.
In progress
Forms with answers saved but not yet submitted — shown with a “Draft saved” badge.
Submitted
Forms you’ve already completed.
Filling out a form

Open the form
Tap any form in To complete or In progress.

Review the prefilled details
Fields mymeddiCAL already knows — your child’s name, date of birth, and (for camp forms) the camp name, location, and dates — are filled in for you.

Complete what's missing
Fill in any remaining fields.

Sign and submit
Read the declaration at the bottom (if present), enter your name in the signature field, and tap Submit.
Filling out a form by email
You can also respond to a form request from your email inbox — no app required:- Open the email from mymeddiCAL.
- Click the link in the email.
- If you’re not already signed in, you’ll be asked to verify your identity with a one-time code.
- Follow the same steps above to fill out and submit.
What happens after you submit
- Your submission is recorded immediately, and the form shows as Completed/Submitted in the school’s system and in your app.
- Reopening a submitted form shows your responses as read-only — you can’t resubmit or edit it.
- If the school sends the form again (for example, a fresh camp), a new copy appears in To complete.
If your child has two guardians and the school sends the form to both, only one submission is needed — once either guardian submits, the form is marked complete for both.
