Send, collect, and track forms — all in one place 📋

Managing student health forms has always meant chasing paper, waiting on emails, and manually checking whether every family has sent something back. Not anymore. mymeddiCAL now includes a complete digital forms system — from sending a form to a camp full of guardians, through to guardians completing it on their phone, to your team seeing the results the moment they come in.

What’s new

A forms library, ready to go

Open the new Forms section and you’ll find a library of ready-to-use templates. It ships with four Australian health action plans to start:

Asthma Action Plan

Anaphylaxis Action Plan

Diabetes Management Plan

Epilepsy & Seizure Management Plan

Use Find a form with AI to search the library in plain language — type something like “asthma action plan” and get matched to the right template. More templates are added regularly.
Uploading your own paper forms for AI digitisation, and building a form from scratch with a drag-and-drop builder, are both coming soon. You’ll see their tiles light up on the Forms page in a future update.

Send a form from the camp Communications tab

Any form in your library can be linked to a camp or excursion. Once linked, it appears as a column in that camp’s Communications tab — right alongside attendance permissions and health plan requests.

Open the camp

Go to Camps, select a camp, and open the Communications tab.

Select students

Check the box next to each student you want to send the form to.

Send

Click Send and confirm. Each guardian gets an email and a push notification.
If a student has more than one guardian, a +1 chip appears next to the primary guardian’s name — click it to include a second guardian in the send.

Track every response in real time

The Communications tab shows a live status for every form, for every student — Not sent, Pending, or Completed. When a form comes back, click it to open the full response immediately — no inbox hunting, no attachments to download. The Received forms panel on the Forms page rounds up every completed submission across your whole organisation in one searchable list.

A dedicated Forms tile in the mobile app

Guardians now have a home-screen Forms tile showing how many forms need attention. Tapping it — or a bell notification — opens a simple page with three sections:

Forms with a saved draft — pick up right where you left off.

Submitted

Forms you’ve already sent back.
Every form opens prefilled with what mymeddiCAL already knows — your child’s name, date of birth, gender, and camp details — so guardians only fill in what’s missing. Submit, and the form closes automatically and drops straight into Submitted.

Camp attendance permission, now in the app too

Approving or declining a camp attendance request still works exactly as before by email — no login required. Guardians can now also respond from the Forms tile in the app. Whichever channel they use, the response is recorded instantly and shows as done on both — only one answer is ever needed.

Security & reliability improvements

This release includes tightened access controls across the forms system: form data can only be viewed or submitted by the guardian it was sent to, submissions are locked to the signed-in session, and deleting a form template preserves every response that was already submitted against it.
